Objective 2

Objective: Collaboratively, to

  1. transform consenting CDPM initiatives linked with PC reform identified in Objective 1, by aligning them to both identified success factors and to promising innovations on patient-centred care for multimorbidity patients;
  2. evaluate qualitatively the transformed programs (study 2.1)
  3. evaluate quantitatively the effects of the transformed programs by using our evaluation framework on outcomes in at least two jurisdictions and compare across jurisdictions (study 2.2).

After completion of the Realist Synthesis and using a collaborative approach, PACE in MM Team will engage the interested CDPM initiatives in an iterative, shared decision-making process of transformation with one selected site in each province Quebec and Ontario.

STUDY 2.1:

Qualitative evaluation of the transformed programs (Measurement & Qualitative sub-committee; Year 2 & 3)

Objective 2b: to evaluate qualitatively the transformed programs.

STUDY 2.2:

Evaluation of effects of the transformed programs (Measurement & Quantitative sub-committee; Years 2 - 5)

Objective 2c: to evaluate quantitatively the effects of the transformed programs by using our evaluation framework to study patients with multimorbidity.

Deliverables for Objective 2:

Valid cross-jurisdictional comparisons of innovative CDPM programs using a rigorous evaluation framework including HA data and with an emphasis on measures of the patient experience.
